I've recently started taking a backpack to the stand with me each time i go out, just curious what all you guys/gals take out with you each and every time.

Depends on the day and the plan. Most hunting is done out the back door so I am lucky if I remember a knife or underwear. Figure if I need it, I walk 10 minutes back.

It depends where I'm at, and if it's a half day or full day hunt, and what the weather forcast is. I've been taking along a backpack for a few years now.
Here's from a morning 'till night stand hunt This is whittled down for a shorter hunt, one closer to home, or a spot and stalk hunt.
Transportation tag(s) zip ties to tie them on field dressing knife zip loc bags (heart and liver) drag rope if in an area that it's needed Towel or baby wipes to clean hands/forearms bottle of water granola bars or trail mix TP (just in case) extra flashlight book rangefinder binoculars possibles bag or a couple extra rounds (I still take them even tho I've never needed them - watch, I will this year now!) lightweight small pruners for cutting small branches Grunt call tree umbrella (if rain or snow is in the forcast)
Cell Phone is in upper jacket pocket, wallet in pants pocket with license.

Pretty well covered it there.
Figured I might be the first to include a book, but you beat me to it.
I'd add my parachute cord (mine's not the real thing...the imitation stuff from Walmart is pretty good and very cheap), and my small camera (my cell phone just makes and receives calls...doesn't play music, take pictures or show movies).
Oh, and a police whistle is a good idea. Have never needed it yet, but it's capable of attracting attention at a greater distance than shouting, and with less chance of being misunderstood. Could be a very small and light lifesaver if you're ever the subject of a search. No batteries needed, and I don't know how you could ever break one.

Blble, a book or magazine, extra rounds, tags, flashlight, gloves, TP, goody powder, thermos of coffee, and a snack to eat, along with an empty coke bottle (you know-especially with after that thermos of coffee), and some empty zip lock sandwich bags. I like to be comfortable and stay at at least 4-5 hours per sitting.

Way to much crap.
At least one book and usually two. A pocket solitaire game. 2 flashlights. Allen wrench set. Extra release. Key ring with keys to all my treestands. GPS mounted to one shoulder strap. Extra battery for my cell phone. Folding saw. Pruners. 2 water bottles. Rain suit. Reflective tacks. Range finder. Binoculars. Extra pull strings. Taurus .40 TP Small bottle of scent killer Various snacks depending on how mong I am planning on being out. <edit> forgot my knife.
Those are the things that live in the pack all deer season long. A few items make appearances depending on the time I am using it. Thermacell and extra fuel/pads. Shells/powder/bullets if I am gun hunting.

binoculars box of shells several extra duck and goose calls canteen of water thermos of coffee (cream and sugar) dog treats duck hanger extra pair of cheap jersey gloves ( I don,t wear gloves when I am hunting) rubber trapping gloves extra decoy line with attached anchor small plastic bag with a few candies a small towel Oh did I tell you I don't Carry a pack when I deer hunt........this is my day pack for duck hunting.

What do you carry to write on your tag with? I use a pencil. I have quit carrying a day pack or hunting bag. I have enough pockets to carry the minimum equipment. I need. Just the essentials. A Cold Steel Master Hunter hunting knife, a shipping tag with the wire tie for tagging the deer, the afore mentioned pencil, a 6' piece of 1/2 in rope, a package of guttin gloves, and for an all day stay a small bottle of water. I have 3 extra loads for my rifle either muzzleloader or PCR, and my cell phone for emergencies. Oops, forgot TP if staying all day.

Stuff that stays Knife Saw Pruners Extra gloves (saves climbing down when I drop one or they get wet) TP with some glass cleaner/fog proof Heart/liver bag Flashlight Halls cough drops, Honey-Lemon Chemical hand warmers Drag rope Binocs Tags w/pencil stub Saran skin to cover flintlock in case of rain Blue Tip Matches in an old marbles match safe for luck. Was my Dad's and going out without it would just not be right Bottle of water Compass, see match safe
Added as needed umbrella and or rain coat Sandwiches snacks spare shells when I carry suppository gun
I carry everything I need to care for and feed my Muzzle loader in a possible bag and powder horn.

I just started carrying a bag this year too. Used to always use a climber, so I'd just have a fanny pack. I started using ladders this year, and realized I could take more in a bag.

I keep my stuff in my "bowbag". I have an extra release, a bottle of windchecker, small binocular, facemask, extra golves, multitool, grunt tube, rangefinder, large hunting knife, small hunting knife, trail marking ribbon, mouth blown coyote call. Everything else I may need is in my vehicle.
